Business templates: route plan, leave request form and service slip

Business templates are a set of three free forms you can use without signing up: a route plan for field visits, an employee leave request form and a service slip filled in after a service call. Each template's fields and what goes into them are in the table; you can print the blank form or copy the table into your own file. The templates hold no preset figures.

What is each template for?

A route plan shows which customers the sales and field team visit, on which day and in which order. A leave request form brings the employee's request, the manager's approval and the HR record onto one sheet. A service slip records the work done, the parts used and the customer's acceptance of the work. The quote template has its own page: you fill it in in your browser with items, quantities and VAT rates.

Which fields go into a route plan template?

A route plan shows which customers a field or sales representative will visit in a given period, and in what order. The template works for a daily or weekly route; in a weekly plan each day is its own row. The rows hold the representative and area, the order of visits, the address and contact, the purpose of the visit, the planned and actual time, and the next step. The fields:

Route plan template
FieldWhat to write
Representative and areaThe name of the person running the route and the area they cover.
Day and dateWhich day the route runs; in a weekly plan each day is its own row.
OrderThe order of visits; stops on the same route are written one after another.
Customer or stopThe firm, dealer or site to be visited.
Address and contactThe visit address and the person to meet there.
Purpose of visitCollection, order, presentation, service or relationship visit, in one sentence.
Planned and actual timeThe planned arrival time and the time the visit actually took place.
Outcome and next stepWhat was discussed, what was taken and the date of the next step.

Printing and copying happen only in your browser; nothing is sent.

How a route plan is built is covered in what is a route plan. Corex360 CRM has activity centre, meetings, appointments, notes and agenda screens; the field team's working day is on the field and operations page.

What goes on a leave request form?

A leave request form brings the employee's request, the manager's decision and the HR record together in one place. It holds the type of leave, the start and end dates, the return date, the total days, the cover, the employee's signature and the manager's approval. For annual leave, weekly rest days and public holidays are not counted in the leave period; for other types your workplace rules apply. The form has these fields:

Leave request form template
FieldWhat to write
EmployeeName, department and role.
Type of leaveAnnual leave, excused leave, unpaid leave or another type defined by your workplace.
Start and end dateThe first and last day of the leave.
Return dateThe day the employee returns to work.
Total daysThe number of leave days; for annual leave, weekly rest days and public holidays are not counted in the leave period, and for other types of leave your workplace rules apply.
CoverThe colleague who takes over the work during the leave.
NotesThe reason for the leave or other information, if needed.
Employee signature and dateShows that the request was made by the employee.
Manager approvalThe name, decision, signature and date of the manager who approves or rejects.
HR recordThe date the leave was entered in the personnel file and the leave schedule.

Printing and copying happen only in your browser; nothing is sent.

You can work out annual leave with the annual leave calculator. In the Corex360 Human Resources module a leave request is opened inside the product, goes to the pending approvals, and the approval is recorded with who gave it and when; the leave calendar is in the same module. Leave types and durations depend on the Labour Law and your employment contract; for a definite answer consult your accountant or legal adviser.

What is written on a service slip?

A service slip records work done on site or in the workshop: what was requested, what was done, which parts were used and whether the customer accepted the work. Each slip has its own number, and it also holds the product's brand, model and serial number, the warranty status, the start and finish time, the signatures of the technician and the customer, and the date of the next service. The slip's fields:

How service work is managed end to end is covered in field service management. Corex360 CRM has a customer support request screen; product warranty periods are also tracked in Corex360. The CRM screens are on the Corex360 CRM page.

Related questions

How do leave, advance, and travel allowance requests work?
Leave, advance, travel allowance, or payment requests are opened in the product and land on the awaiting-approval screen, where the manager approves or rejects them. Approved leave and advances sit on the employee card. Every approval is recorded with who and when; no verbal approval or paper signature is needed, and the request does not get lost in WhatsApp. How this differs from a paper form is in leave on paper or digital.
How is visit order built?
First customers are grouped into territories, then each gets a visit frequency, and finally the day's order is set by road layout and the customer's hours.
What is a route plan?
A route plan is a recurring visit schedule that sets which days and how often a field rep visits each customer. In distribution, wholesale, and dealer visits it gives the field team its weekly rhythm.
How is a work order opened, and what goes into it?
A work order is a task card that says on one record what is to be done, where, by whom, and by when.
What is the difference between a work order and a service form?
A work order is opened before the job: what, where, who, when. A service form is filled in after the job: what was done, which parts were used, what the customer said. When both are tied to the same record, the service history builds itself.
